DSO Event Proposal Form

  • This form must be submitted at least two weeks (10 business days) prior to the proposed date of the event.* Once submitted, the event will be reviewed within 2-4 business days. If denied, students will be given feedback, and offered the opportunity to re-submit.

  • Please list any groups that will be collaborating on or co-hosting this event.
  • Event Overview

  • Please briefly describe the event.
  • All post-event assessments are required no later than one-week following the event. Example, if your event is held on Tuesday at 6pm, your post-event assessment is due to the Division by the following Tuesday at 6pm
  • Event Details

  • Select the option that most closely applies.
  • Ex. September 30, 11:30am-1pm.
  • If multiple spaces are needed, please describe in detail what each space will be utilized for.

  • Make sure to include guest speakers in addition to attendees.

  • Reminder: Meals for student events must adhere to the limits of $25 (breakfast/lunch), $45 (dinner) per person.
